Why We Don’t Offer Exosome Treatments — Doctor-led Alternatives in Worthing

You may have started seeing exosome treatments advertised around Worthing and across Sussex. They’re being marketed as the next big thing in skin regeneration — and the science behind them is genuinely interesting. So why don’t we offer them here at Blue Bird Aesthetics?

The honest answer is the same one I give about a handful of other fashionable treatments: I’m not yet satisfied that the evidence, the regulation and the safety data are where they need to be for me to put my name to it. As ever, I’d rather tell you exactly what I think and why — so here is my full reasoning, and you can make up your own mind.

First, what exosomes actually are

Exosomes are tiny vesicles — think of them as microscopic parcels — that cells release to communicate with one another. They carry proteins, lipids and genetic material, and in theory they can signal nearby skin cells to repair and regenerate. It’s an elegant idea with real laboratory research behind it, and I want to be fair about that. Exosomes aren’t a gimmick, and one day a properly regulated, well-evidenced exosome treatment may well earn its place. We’re simply not there yet.

The regulatory reality in the UK

In the UK, the MHRA classifies exosomes intended for injection as medicinal products. At present, no exosome injectable holds a UK marketing authorisation, which means they cannot be supplied or administered as an aesthetic treatment. Human‑derived exosomes are not approved for cosmetic use, and there is currently no authorised product that meets UK safety, quality or regulatory standards for injection into the skin.

At present, exosomes can only be applied topically in this country — usually painted onto the skin after microneedling. So if you’re ever offered exosomes as an injection, that is a genuine red flag and worth questioning carefully. It’s also telling that, because the area sits in a regulatory grey zone, many practitioners’ standard insurance won’t cover exosome use at all. When the wider profession is this cautious about cover, I take that seriously.

The evidence is promising, but early

The studies we do have are encouraging in places — small trials report improvements in skin tone and texture and high patient satisfaction. But “promising and early” is not the same as “proven and safe.” There are still no standardised treatment protocols, very little long-term safety data, and published case reports of serious reactions — including persistent lumps and granulomas — after unregulated injectable exosome products. For a treatment I’d be recommending to you, that gap matters. I want to know not just that something can work, but that it works reliably and is safe over years, not weeks.

And then there’s the question of where they come from

This is the part that gives me the most pause, and it’s rarely spelled out clearly to patients. The exosomes used in aesthetics are derived from a surprisingly wide range of sources. Some come from human stem cells; but many on the market come from animals — bovine colostrum and milk, stem cells from the umbilical cords of red deer, even salmon (including salmon sperm and caviar) — or from plants such as rose, ginseng and aloe.

Here’s my concern as a doctor: a signalling parcel built by a cow, a deer, a salmon or a rose is not the same as one built by a human cell. Even the animal-derived exosomes that are closest to us don’t match human cell receptors perfectly, and the long-term consequences of introducing this kind of cross-species biological material into your skin simply aren’t well understood yet. Before I’d ever offer a treatment, I’d want to know precisely what is going into your skin and what it does over time. For a lot of the products being sold right now, those answers aren’t clear enough for me.

What we offer instead

I’m not against exosomes on principle. I’m against offering anything before the evidence, the regulation and the safety data have caught up — and I keep all of this under active review. If and when a properly regulated, robustly evidenced exosome treatment arrives, I’ll happily revisit this page and say so openly.
