In Short

  • Volume loss is three-dimensional — bone, fat and soft tissue all change simultaneously.
  • The cascade effect: cheek volume loss often worsens the lower face even when the jaw itself has not changed.
  • Conservative staged treatment produces better long-term results than large one-off volumes.
  • Skin boosters are not a substitute for filler here — they address different layers.

What Is This Concern?

Volume loss is a three-dimensional process. Patients describe it as: “I look older than I feel”, “my face has changed shape”, or “I look tired even when I am not.”

What Causes It?

  • Bone resorption — facial bones remodel with age, reducing the structural foundation everything rests on.
  • Fat compartment deflation — discrete facial fat pockets deflate and descend independently.
  • The cascade effect — mid-face volume loss reduces support for the lower face, worsening folds and early jowling even without those areas changing directly.
  • Collagen thinning — the dermis becomes thinner and less dense.

Treatments That Help

Dermal fillers — the primary treatment

HA filler placed conservatively at the right depths. Approach depends entirely on individual assessment.

Profhilo Structura

For early volume change alongside skin quality concerns — deep bio-remodelling with subtle structural support.

What Will Not Help

  • Skin boosters — improve hydration and quality but do not restore structural volume.
  • Anti-wrinkle injections — address muscle movement, not volume loss.

Frequently Asked Questions

How do I know if I need filler or something else?

The clearest sign is a genuine loss of facial fullness — cheeks that were plump and now look deflated, or a face that has visibly changed shape without weight change. If your concern is primarily skin quality or dullness, fillers are unlikely to be the right first step.

Can skin boosters replace filler for volume?

No. Skin boosters work at a different depth — they improve hydration and quality rather than restoring structural volume. They are complementary, not interchangeable.

Why do my cheeks look flat when I have not lost weight?

Facial volume loss is driven by ageing-related changes to fat compartments and bone — not by general body weight. Facial deflation while body weight is stable is entirely common.

Will filler make me look unnatural?

Conservative, staged treatment aimed at restoring your own natural proportions should not. The over-done look results from excessive volume, accumulated filler without review, or poor anatomical placement.

How long does filler last?

Cheek and structural filler: 12-18 months. Lip filler: 6-12 months. Individual metabolism, product choice and area all influence duration.
